Accident Summary:
THE AIRCRAFT, SN: 65-9433, WAS SUPPLYING GUN COVER FOR A COMBAT OPERATION NORTHWEST OF PLEIKU, RVN AND WAS UTILIZING CAMP HALLOWAY FOR FUEL NEEDS. THE AIRCRAFT HAD 950 POUNDS OF FUEL ABOARD AT TAKE-OFF, A LOAD OF 142.75 INCH ROCKETS, 2000 ROUNDS OF 7.62MM FOR THE MINI-GUNS, AND 1000 ROUNDS OF 7.62MM FOR THE DOOR-MOUNTED M-60'S. AFTER REFUELING, THE AIRCRAFT COMMANDER, CPT HOLT, HAD TO MAKE SEVERAL ATTEMPTS TO HOVER THE AIRCRAFT FROM POL TO THE RUNWAY AND WAS UNSUCCESSFUL IN EACH ATTEMPT, AND HAD TO SET THE AIRCRAFT DOWN. THE AIRCRAFT COMMANDER THEN INSTRUCTED THE CREW CHIEF AND GUNNER TO EXIT THE AIRCRAFT UNTIL HE HAD IT LINED-UP ON THE RUNWAY FOR DEPARTURE. AFTER REPOSITIONING THE AIRCRAFT ON THE ACTIVE AND BACK-TAXIING THE AIRCRAFT COMMANDER HAD THE TWO CREW MEMBERS GET BACK ON BOARD. THE AIRCRAFT COMMANDER THEN REPORTED HOVER CHECK COMPLETE AND THAT HE WAS READY FOR DEPARTURE. THE TOWER REPORTED THE WINDS TO BE OUT OF THE NORTHEAST AT 7 KNOTS AND GAVE THE AIRCRAFT CLEARANCE FOR DEPARTURE. THE AIRCRAFT COMMANDER DECIDED TO MAKE A MINIMUM POWER TAKE-OFF. UPON TAKE-OFF THE PILOT, WO1 DEVERE, INFORMED THE AIRCRAFT COMMANDER THAT THEY HAD 6600 RPM AND THAT THE INSTRUMENTS WERE IN THE GREEN. SHORTLY AFTER TAKE-OFF THE INSTRUMENTS WERE READING 6300 RPM, 94% N1, AND 36 POUNDS OF TORQUE. THE AIRCRAFT SEEMED TO BE HOLDING AT THESE READINGS SO THE AIRCRAFT COMMANDER LOWERED THE COLLECTIVE AND NOSED THE AIRCRAFT OVER TO BUILD UP MORE AIRSPEED. PRIOR TO CROSSING OVER THE FENCE AT THE END OF THE RUNWAY THE AIRCRAFT WAS AT 50 - 55 KNOTS OF AIRSPEED, IN TRANSLATIONAL LIFT, AND HAD CLIMBED TO AN ALTITUDE OF APPROXIMATELY 30 FEET. JUST AS THE AIRCRAFT PASSED OVER THE FENCE AT THE END OF THE RUNWAY THE RPM BLED FROM 6300 TO 5100 AND THE AIRCRAFT BEGAN TO LOSE ALTITUDE. THE AIRCRAFT COMMANDER INFORMED THE CREW THAT THE ENGINE WAS LOSING POWER AND TO PREPARE FOR A CRASH LANDING. THE TERRAIN IN FRONT OF THE AIRCRAFT WAS UNEVEN WITH DIRT MOUNDS AND DITCHES. CPT HOLT HELD THE COLLECTIVE STABLE AND KEPT THE AIRCRAFT STRAIGHT AND LEVEL. PRIOR TO CONTACT WITH THE GROUND THE AIRCRAFT COMMANDER PULLED PITCH BUT THERE WAS NO NOTICABLE RESPONSE. THE AIRCRAFT HAD A HARD LANDING AND THE LEFT SKID HIT A MOUND OF DIRT THUS FORCING THE AIRCRAFT BACK INTO THE AIR WITH A ROLL TO THE LEFT. THE AIRCRAFT COMMANDER PULLED THE CYCLIC TO THE RIGHT AND TO THE REAR. THE TAIL OF THE AIRCRAFT HIT THE GROUND CAUSING THE MAIN ROTOR BLADES TO FLEX DOWN SEVERING THE TAIL BOOM. THE AIRCRAFT CONTINUED TO ROLL IN A CLOCKWISE DIRECTION AND FINALLY CAME TO REST ON ITS LEFT SIDE.\\
